EMPLOYER INDEMNITY COVERAGE PLAN HIGHLIGHTS
• $1,000,000 Combined Single Limit per person per occurrence
• Choice of $1000, $2500 or $5000 Deductible with Benefit
   Periods of 117 or 156 Weeks
• Occupational Accident Benefits that cover employees hurt on the
   job (Accident Medical Benefits, Weekly Disability Benefits, and
   Accidental Death & Dismemberment Benefits)
• Occupational Disease/Cumulative Trauma Coverage and Waiver of
   Subrogation included
• Employer Indemnity Coverage that Covers:
       a. Amounts Paid to settle a claim for workplace negligence
           brought by a covered employee as a result of a covered
           injury.
       b. Amounts Paid to satisfy a judgment for an employer
           indemnity claim.
       c. Defense Costs and Expenses related to an employer
           indemnity claim.
       d. Defense Costs are paid in addition to the $1,000,000
           CSL Policy with no maximum.

Plan Details

• $1,000,000 Combined Single Limit Benefit Includes;
       a. Accidental Death & Dismemberment benefits up to the lesser of
           10x annual salary or $500,000
       b. Weekly Accident Indemnity benefit equal to 75% of salary to a
           maximum of $600 per week (subject to a 7 day elimination period;
           benefits retroactive to 1st day)
       c. Accident Medical Expense Benefits

 
• Benefit Period options of 117 weeks or 156 weeks
 
• Deductable options of $1,000; $2,500; or $5,000 per person per accident (deuctable applies to disability and accident medical benefits)
 
• Coverage includes losses resulting from Occupational Disease or Cumulative Trauma
 
• Waiver of Subrogation provision included in policy

Groups of 2 to 50 lives that generate less then 30% of their revenue from reposession operations;

Underwriting Guidelines:
 
1. There is a 2 life minimum; one life groups are not eligible.
2. Contract laborers (1099) are not eligible for coverage.
3. Groups must have a three-year loss ratio of 50% or less to be eligible for the rates outlined above. For a firm quotation, please provide us with documented loss data for the last 3 years, along with the following information:
 
a. Your company names as it appears on your federal filings;
b. Your federal tax Identification number;
c. The percentage of revenue generated from reposession operations; and
d. The number of W-2 employees in each rating category listed above.

 
4. Any group that exceeds 50 employees must be referred to the administrator prior to receiving approval to use the program rates.
